[ ] DV-101 DESCRIPTION OF ABUSE

[ ] IF DV-101 DOES NOT HAVE ENOUGH SPACE, YOU MAY TYPE UP MORE INFORMATION. TITLE THE DOCUMENT “ATTACHMENT TO DV-101″ AND ATTACH IT TO THE DV-101 FORM

[ ] EXHIBITS: IF YOU HAVE ANY EXHIBITS TO SUPPORT YOUR FACTS AND CONTENTIONS, LABEL YOUR EXHIBITS WITH NUMBERS (ON THE BOTTOM OF THE EXHIBIT).              AFFIX      EACH NUMBERED-EXHIBIT TO YOUR DV-101 FORM

[ONLY FILL OUT SECTIONS 2 AND 2, AND THE COURT INFORMATION/ CASE NUMBER]

[ ] DV-140 CHILD CUSTODY AND VISITATION ORDER
[OPTIONAL: CASES WITH CHILDREN: IF YOU ARE REQUESTING CUSTODY ORDERS IN YOUR CASE BY FILING THE DV-105 FORM, YOU MUST ATTACH THE DV-140 FORM TO THE DV-110 FORM]

[ ] DV-145 ORDER NO TRAVEL WITH CHILDREN
[OPTIONAL: CASES WITH CHILDREN: IF YOU ARE REQUESTING ANTI-CHILD ABDUCTION ORDERS IN YOUR CASE BY FILING THE DV-108 FORM, YOU MUST ATTACH THE DV-145 FORM TO THE DV-110 FORM]

[ ] FL-150 INCOME AND EXPENSE DECLARATION
[OPTIONAL: THIS FORM IS ONLY REQUIRED IF THE PERSON SEEKING THE RESTRAINING ORDER IS REQUESTING CHILD SUPPORT, SPOUSAL SUPPORT, OR ATTORNEY’S FEES. FAILURE TO FILE THIS FORM DOES HAVE CONSEQUENCES. MAKE SURE TO ATTACH EITHER YOUR PAYSTUBS OR A PROFIT AND LOSS STATEMENT TO THIS FORM AND BRING YOUR LATEST TAX RETURN TO COURT]

*Please note: There are additional domestic violence forms utilized for a variety of post-filing and post-response issues, including but not limited to forms to Request a Continuance of a Hearing and forms for Post-Hearing Orders. I have not mentioned these forms above as they are not needed at the preliminary-filing stage.
